Overall: updated font/size helps

PAC section

Section C. PAC ethnicity/racial diversity
[not sure what the vacancies column is about.
total PAC members serving on 12/30 [date] should equal
     B.10.a
The next page asks for totals by gender doesn't reference
      must agree with B.10.a

Section E. 1. For state-operated P&A Programs Only:
   question (a) Is this a State-operated P&A System? Yes/No
   [if this section is being filled out the answer must be YES;
consider moving the question to just prior to this section]

Section F. 1. Are P&A program staff invited to attend PAC meetings? & Section F. 2. Were other governing board members invited to PAC meetings?

Interesting question about 'usually invited to attend'.
It's easy for P&As to 'invite staff', wouldn't actual attendance
    be more useful?

Questions F.4 and F.5 [This appears to be the same question asked twice]

It's nice that the regulation saying this is optional is provided.
Consider making clear that doing so was optional, but please answer the
question. It also might be more useful for each activity being listed
to describe if is in-state or out-of-state.

F.6 Does the P&A...

Very long question. If don't know; not sure what additional explanation
    would be needed.

    [error in numbering questions]
    There is a question F.6 on p 8. and a different question F.6 on page 9. !!

F.7 remimbursement of expenses

Many P&As report many activities; perhaps this is best answered
     by an attachement

Q F.9.A 1. Does the P&A have procedures established for public comment?
[this is a question in it's own right] This is followed by
            parts a, b and c.

Comments on PPR questions

Fonts and larger print are nice.
Table of contents is nice.

Looks like a lot more information is being collected.

I like the tables for Section 4.A.1, 4.B.1 and 4.C.1
please make clear if you want just check boxes for outcomes A,B,C,D
or actual numbers. If numbers, please make sure if each problem
can have more than one outcome and that the totals at the end need
   not add up exactly.

^.C.4 Dissemination activities

I found this table very confusing to fill out and not sure
how well the outcomes match this table

8.E. If the answer is no, please provide a brief explanation
          There were 4 yes/no questions
